Consent Forms

Release of Liability and Publicity Consent Waiver:
In order to particpate in the Kids Walk for Kids with Cancer, all walkers must complete and submit a hard copy of a Release of Liability and Publicity Consent waiver to Memorial Sloan-Kettering Cancer Center. Download your form or complete the waiver included in the Kids Walk brochure. You can return the waiver to Memorial Sloan-Kettering, or bring it with you to the Kids Walk. If you are under 18 years old, your parent or guardian will have to complete it.

HIPAA Release Form:

MSKCC has a responsibility to protect patient health information. The HIPAA release form is an agreement that walkers will not publish their own health information or someone else’s on their online fundraising pages or in any e-mail or letter seeking contributions, unless they provide MSKCC with a completed HIPAA release form.

Release of Liability and Publicity Consent waivers and HIPAA release forms will also be available at the Check In table during the Kids Walk for event day registrants. Please note, a parent or guardian must sign off on the documents if the walker is under 18 years old. We encourage these participants to download the form ahead of time and/or come with one already completed from the Kids Walk brochure.
